My Health Patient Portal - Requesting & Accessing Health Information

Use the MyHealth Patient Portal to access your medical record anytime anywhere. 

Anderson Healthcare offers a patient portal to access an electronic copy of your medical record. Keep yourself in the know of your medical care with Anderson by viewing medical record reports, results, appointments immediately as they become available. Users can self-enroll if they already have an email address on file with us by clicking the following link:

Use Google Chrome when accessing the portal via an internet browser or download the Meditech MHealth smartphone app from the Google Play or Apple App store!

If your email address is not on file with us, please complete the Portal Enrollment Form. An email from [email protected] will be sent to you once your request has been processed for you to complete your enrollment.

Share your portal with familly, friends or other members a part of your care, via the Shared Access routine once you logged in. Manual proxy aeeess ean also be complete by our MyHealth portal team by completing the Portal Proxy Form.

Parent/Guardian Proxy Access Instructions

• Children younger than 12 years old- the parent/guardian should sign as both the proxy and patient.
• Children 12 years and older- the child should sign as the patient This is to comply with IL State and Federal Laws which allow minors 12 years and older to consent to treatment for particular types of services. In which case, related medical information may not be permissible to release to a parent, guardian or proxy. The MyHealth patient portal does not limit particular types of information; therefore, we must abide by legal requirements of obtaining consent from both the minor patient (age 12 and older) and proxy.

*In situation of multiple children, each child will require their own proxy form.

While records are available at all time through the portal we understand that there may be a neee for a printed version. The forms below can be completed to begin this records request process.
